Please complete Part A and Part B in order for us to expedite your Additional Visa Pages to your current U.S. Passport application process:

PART A - PREPARING APPLICATION AND SUPPORTING DOCUMENTATION. Print this page for ease of reference

1. Current U.S. Passport – You must submit your valid signed U.S. Passport which is not expired.

2. Complete Application Form DS-4085 online for Additional Visa Pages – Click here to download DS-4085 Form. We can only accept online DS-4085 application fom.
   • Complete details of your travel itinerary/tickets in “Travel Plans” section 12.
   • Print a copy of the completed application form.Sign .

3. Proof of Departure – Your original travel itinerary or a copy of your travel ticket. In order to expedite your passport application, your travel must start within 2 weeks from the date of application.

4. Authorization Letter – Please download the Authorization Letter and complete two copies of the letter (according to the information contained in your passport), and sign both copies where indicated.

5. Fees
   1. Government Expedite Fee $142 –You should pay this with a personal check, company check or Money Order - payable to "U.S. Department of State. (write your full name and Date of birth at the top of the check)Our Service Fee $100.00 – Payable to “Global VIP Services”
   2. FedEx Return Mail Service – See Order Form for details
6. Order Form – Complete and sign VIP Services Order Form.

PART B - HAND IN/SHIP APPLICATION & RELATED DOCUMENTS TO VIP SERVICES

Post the following items to us at the address below via FedEx overnight delivery service:-

• Order form (completed and signed) - original
• Your current signed passport - original
• Your completed and signed Additional Visa Pages Application Form DS-4085 - original
• Letter of Authorization (completed and signed) – original x 2
• Your driver’s license - a clear copy x 2
• Proof of Departure – copy x 2
• Payment:
  • Government Fee – A check for $142.00 made payable to U.S. Department of State
  • Complete your credit card details on the Order form, or enclose one company check, Money Order made payable to GLOBAL VIP SERVICES for $100, provided that you include a return mail FedEx airbill, otherwise your payment to Global VIP Services should include the FedEx Return Mail Fee as well.
