If your passport is stolen or lost, you must report the loss of your valid passport and apply to replace your passport.

PART A - PREPARING APPLICATION AND SUPPORTING DOCUMENTATION

  1. Complete U.S. Passport Application Form DS-11 online. We can only accept online DS-11 and DS-64 applications – Click here to download DS-11 Form and DS-64 Form.

    Complete details of your travel itinerary/tickets in “Travel Plans” section.Please complete “Emergency Contact” section. This is extremely helpful should you have any difficulties overseas and need assistance from a U.S. Embassy or Consulate.

    • Print a copy of the completed application forms (DS-11 and DS-64). Do Not sign the forms until you are in the presence of a “Application Acceptance Agent” or County court Clerk, and instructed to do so – see Part B.

  2. Proof of U.S. Citizenship (applicant) - Prove of U.S. Citizenship with any one of the following:

    • Certified US Birth certificate issued by the city, county or state
    • Previous fully valid U.S. Passport (mutilated, altered, or damaged passports are not acceptable)
    • Consular Report of Birth Abroad or Certification of Birth
    • Certificate of Citizenship or Naturalization

  3. Proof of Departure – Your original travel itinerary or a copy of your travel ticket. In order to expedite your passport application, your travel must start within 2 weeks from the date of application.
  4. 2 Color photographs – Photograph Specification.

  5. Present Proof of Identity - You may prove your identity with any one of the following, if you are recognizable:

    • Previous U.S. passport (mutilated, altered, or damaged passports are not acceptable)
    • Naturalization Certificate
    • Current, valid
      1. Driver's license
      2. Government ID: city, state or federal
      3. Military ID: military and dependents

  6. Authorization Letter – Please download the Authorization Letter and complete two copies of the letter (according to he information contained in your passport), and sign both copies where indicated. Parent(s)/guardian(s) of children under 16 years of age,must sign both copies below the minor's siganture..

  7. 48-Page Passport - If you travel extensively, you may submit a signed request for a 48-page passport. There is no additional cost for this.

  8. Fees
    1. Government Fee $170.00 – You should pay this with a personal check, company check or Money Order - payable to "U.S. Department of State. (write your full name and Date of birth at the top of the check)Application Acceptance Facility Execution Fee $25.00 – Pay this directly to the Application Acceptance Agent/ County Court Clerk.Our Service Fee $100.00 – Payable to “Global VIP Services”

PART B - APPLICATION ACCEPTANCE FACILITY – PROCEDURE

Present the following items to the authorized agent at a Application Acceptance Facility near you for approval and let the agent know that you are using an expediting service to "hand carry" your application. You must pay $25 application execution fee to the Application Acceptance Agent.

  • Completed Lost/Stolen Passport FormDS-64 - original
  • Completed Application Form DS-11 - original
  • Your signed damaged passport (if applicable) - original
  • Proof of U.S. Citizenship - original
  • Proof of Departure - copy
  • Two color passport-type photographs
  • Letter of Authorization (completed and signed) -2 originals
  • Your valid driver's license - a clear copy
  • Government Fee – A check for $170.00 made payable to U.S. Department of State
  • In case of a name change - your Marriage/Divorce certificate or Court Order -original
  • Parental Proof of Identity - copy (a clear copy of parent’s drivers license)

You will be asked to show your proof of identity (original) and the agent will ask you to sign the passport application form. Please make sure that the agent notes on the application the type of ID you have presented. The agent will place the above items in an envelope and seals the envelope. The sealed envelope will be marked “To Be Opened by U.S. Passport Office Only”. Do not open this sealed envelope or your application will become invalid.

The agent must return the sealed envelope to you.

DO NOT seal your Order form or the payment to Global VIP Services in the envelope


PART C – HAND IN/SHIP PASSPORT APPLICATION & DOCUMENTS TO GLOBAL VIP SERVICES

Post the following items to us at the address below via FedEx overnight delivery service:

  • Order form (completed and signed) - original
  • The Government sealed envelope.
  • Completed Lost/Stolen Passport Form DS-64 - copy
  • Completed Application Form DS-11 - copy
  • Letter of Authorization - copy
  • Payment – Complete your credit card details on the Order form, or enclose one company check, Money Order made payable to GLOBAL VIP SERVICES for $100, provided that you include a return mail FedEx airbill, otherwise your payment to Global VIP Services should include the FedEx Return Mail Fee as well.
