Please review the following criteria for each passport application, to choose the right one for you. If you are not sure about the type of passport you need or/and supporting documents please contact us.

New Passport / First Time Passport

You must apply for a new passport if you answer yes to one of the following criteria

  • You are applying for a U.S. passport for the first time
  • Your previous U.S. passport has expired & was issued more than 15 years ago
  • Your previous passport is expired and it was issued when you were under 16 years old.
  • Your name has changed since your passport was issued and you do not have a legal document formally changing your name.

Passport Renewal

You must meet all of the following criteria to apply for a passport renewal

  • You can submit your most recent US passport which it is not damaged, altered or mutilated; and
  • You were at least 16 years old when your most recent US passport was issued; and
  • Your most recent US passport was issued less than 15 years ago; and
  • You use the same name as on your most Recent US passport.
OR
  • You have had your name changed by marriage, divorce or court order and can submit proper documentation to prove your name change.

Child’s Passport (Minors under age 14)

If you are under Age 14, you must establish proof of citizenship AND proof of relationship to parents/legal guardians. Previous U.S. passports are not acceptable as proof of relationship to the applying parent(s)/guardian(s).

If you are under Age 16, you must establish proof of U.S. citizenship AND proof of relationship to parents/legal guardians. Previous U.S. passports are not acceptable as proof of relationship to the applying parents(s)/guardian(s).

Lost, Stolen or Damaged Passport

If your valid passport is lost/ stolen/ damaged, you must apply for a replacement of your passport.

Adding Visa Pages to Valid Passport

If you have a current passport which is not expired and the visa pages are full. You can apply for additional visa pages to be inserted to your current passport.

Changes/Corrections to Current Valid Passport

You should ONLY apply if you want to make a data change/extend the validity for a current valid U.S. passport WITHIN one year from issue date

  • Your most recent U.S. Passport was issued less than one year, but more than 30 days ago.
  • You can submit your most recent U.S. Passport.
  • You have had a change of name;
Your identifying information was printed incorrectly;
OR
Your most recent U.S. passport was limited in validity for one year or less and/or was issued abroad.

If you answer NO to any of the above 3 statements, then you cannot re-apply using Form DS-5504. You may apply for a New passport or passport RENEWAL, depending on your circumstances. Contact Us if you need assistance.

Extending a Passport with Limited Validity

To extend a limited validity U.S. passport anytime while the passport is valid, you may apply for passport Renewal if you were over 16 years old when your current passport was issued. Also see Changes/Corrections to Current Valid Passport.

Name Change

Your name has been legally changed due to marriage, divorce or a court order within one year from the date of issuance of your passport, you must use the Re-Application process to get a new passport with your new name

If your name has been legally changed due to marriage, divorce or a court order and it has been more than one year from the date of issuance of your current valid passport and you were over 16 years old when the passport was issued, you must apply for a passport Renewal application

If your name has been legally changed due to marriage, divorce or a court order and it has been more than fifteen years from the date of issuance of your current passport, you must apply for a New passport.

Your name has changed since your passport was issued and you do not have a legal document formally changing your name, you must apply for a New passport.

Second Passport

  • Your current passport must be valid and issued less 10 years ago;
  • You can submit your current US passport which it is not damaged, altered or mutilated;
  • You were at least 16 years old when your current US passport was issued; and
  • You use the same name as on your current US passport.
