The following is a list of passports and their criteria for your review. Once you have determined which type of passport you need to expedite, just select it to see the full step by step instruction and download the relevant forms. We offer to review your scanned passport applications and supporting documents via email, to avoid mistakes and to save time.

Apply for a New US Passport (First Time Passport) if:

  • you have never had a US passport, or
  • your passport was issued more than 15 years ago, or
  • your most recent passport is damaged, altered or mutilated, or
  • your most recent US passport was issued prior to 16th birthday.

If you travel frequently, you may request to be issued a larger passport book with 52 pages, provided that your most recent passport shows that most pages are used (visa stamps, entry and exit stamps).

Apply to Renew your US Passport if:

  • It is undamaged and can be submitted with your application;
  • Was issued when you were age 16 or older;
  • Was issued within the last 15 years; and
  • Was issued in your current name or you can legally document your name change with original or certified copy of your marriage certificate or the government-issued document evidencing your legal name change.

If any of the above statements do not apply to you, you must apply for a New Passport.

If you travel frequently, you may request to be issued a larger passport book with 52 pages, provided that your current passport shows that most pages are used (visa stamps, entry and exit stamps).

Name Change in your passport:

  • you are able to submit your current valid US passport, and
  • you have had your name changed by marriage, divorce or court order and can submit proper documentation to prove your legal name change since your passport was issued, and
  • your valid passport was issued when you were 16 years of age or older, and
  • your valid passport is not altered, damaged or mutilated.

NOTE:

  • If less than one year has passed since your passport was issued, you must apply for a Passport Corrections.
  • If your current passport was issued less more than 1 year ago but less than 10 years, you must apply for Passport Renewal.
  • If your current passport was issued more than 15 years ago, you must apply for a New Passport.

Apply for a second valid US passport if:

  • you are able to submit your current, valid US passport, issued less than 10 years ago, and
  • your current, valid passport was issued when you were 16 years of age or older, and
  • your current passport is not altered, damaged or mutilated, and
  • you can provide proof that you need to have two valid US passports.

Apply for Children Under 16 Passports if:

  • you are under 16 years of age; and
  • you can establish proof of U.S. citizenship AND proof of relationship to parents/legal guardians. Previous U.S. passports are not acceptable as proof of relationship to the applying parents(s)/guardian(s).
    • you can provide proof of parental relationship – certified US birth certificate or birth certificate with full details of the applicant and parents, or certified birth certificate issued by other countries. If the birth certificate is in other language, you must provide certified translation in English, and
  • your parents give their consent. Both parents must accompany you to the post office. If one parent cannot attend they must complete the consent Form DS-3053, have it notarized and a clear copy of their state ID/driver license (front and back) should be submitted by the other parent at the post office.

Minors under 16 passports are valid for 5 years. Every minor regardless of age travelling abroad must have a passport.

Apply for Passports for Children 16-17 Year if:

  • If you are 16Y or 17 years of age; and
  • you can establish proof of U.S. citizenship AND proof of relationship to parents/legal guardians. Previous U.S. passports are not acceptable as proof of relationship to the applying parents(s)/guardian(s).
    • you can provide proof of parental relationship – certified US birth certificate or birth certificate with full details of the applicant and parents, or certified birth certificate issued by other countries. If the birth certificate is in other language, you must provide certified translation in English, and
  • you can provide parental consent by one parent only.

Apply for Replacement of a Lost/Stolen US Passport if:

  • your passport is lost or stolen, and
  • you can provide evidence of US citizenship, and
  • you have a valid government-issued photo identity.

If your passport has been lost or stolen, it should be reported immediately to help protect yourself against identity theft and to prevent someone else from using the passport.

Apply to Correct Passport Errors if:

Printing errors and spelling errors done by the passport office (government), like: incorrect name, gender, or place of birth, can be corrected at any time while the passport is still valid.

  • you are able to submit your current valid US passport, and
  • you are able to provide proof for the correct data, and
  • your valid passport was issued when you were 16 years of age or older, and
  • your valid passport is not altered, damaged or mutilated.

To correct a printing or spelling error in your passport, you must apply for a Re-Application.

NOTES:

  • If reported within one year of issuance, a new, corrected passport will be issued, which will be valid for 10 years from the date the new, correct book is issued.
  • If reported after one year of issuance, the new, corrected passport will be valid until the expiration date of the original, incorrect passport.

The requirements for replacement passports apply for both adults and minors.

Apply to Extend Validity of Limited Passport if:

A limited validity passport is any passport issued with less than full 10-year validity for those 16 years and older and less than 5-years validity for those under age 16. If you have been issued a limited validity passport, the process for obtaining a new full-validity passport depends on how long ago it was issued and where your passport was issued.

If your passport was issued domestically (in the United States) and was limited due to a citizenship, identity, name, or gender documentation deficiency, and was issued:

 You must submit documentary evidence to enable the Passport office to extend the validity of your passport.
