PART A - PREPARING APPLICATION AND SUPPORTING DOCUMENTATION
- Complete U.S. Passport Re-Application Form DS-5504 – Click here to download DS-5504 Form
- Complete details of your travel itinerary/tickets in “Travel Plans” section.
- Please complete “Emergency Contact” section. This is extremely helpful should you have any difficulties overseas and need assistance from a U.S. Embassy or Consulate.
- Print a copy of the completed application form.Sign and date it.
- Most Recent Passport – Submit your signed current valid passport.
- Proof of Departure – Your original travel itinerary or a copy of your travel ticket. In order to expedite your passport application, your travel must start within 2 weeks from the date of application.
- 2 Color photographs – Photograph Specification.
- Evidence To Support a Name Change/Other Changes in Descriptive Data:
- Name Change - If your name has changed, you must submit a marriage certificate, divorce/adoption decree, or court order showing the name change.
- Change or error - If there is a change or an error in your current passport, you must submit the appropriate evidence showing the correct information (e.g. birth/marriage certificate).
- Evidence To Extend Passport Validity - If you want to re-apply because your U.S. passport is limited in validity due to a lack of citizenship evidence or identity, you must submit: the necessary document to prove this (your birth certificate or certificate of citizenship) and a letter explaining the reason(s) that lead to a limited validity passport being issued to you.
- Authorization Letter – Please download the Authorization Letter and complete two copies of the letter (according to the information contained in your passport), and sign both copies where indicated.

Post the following items to us at the address below via FedEx overnight delivery service:-
- Order form (completed and signed) - original
- Your signed current passport - original
- Completed and signed Application Form DS-5504 - original
- Proof of Departure – copy x 2
- Two color passport-type photographs
- Letter of Authorization (completed and signed) – original x 2
- Evidence To Support:
- Name change – In case of name change, submit your Marriage/Divorce certificate or Court Order - original
- Change/Error in your passport – Submit your birth/marriage certificate - original
- Request to extend your passport validity – Submit your U.S. citizenship proof (birth/Citizenship certificate) – original
- Your driver’s license - a clear copy x 2
- Payment:
- Government Fee – A check for $60.00 made payable to U.S. Department of State
- Complete your credit card details on the Order
form, or enclose one company check, Money Order made
payable to GLOBAL VIP SERVICES for $100,
provided that you include a return mail FedEx airbill,
otherwise your payment to Global VIP Services should
include the FedEx Return Mail Fee as well.
