PART A - PREPARING APPLICATION AND SUPPORTING DOCUMENTATION

- Complete details of your travel itinerary/tickets in “Travel Plans” section.
- Please complete “Emergency Contact” section. This is extremely helpful should you have any difficulties overseas and need assistance from a U.S. Embassy or Consulate.
- Print a copy of the completed application form.Sign and date it.
- Most Recent Passport – Submit your signed previous passport. This will be returned to you with your new passport.
- Proof of Departure – Your original travel itinerary or a copy of your travel ticket. In order to expedite your passport application, your travel must start within 2 weeks from the date of application.
- 2 Color photographs – Photograph Specification.
- Proof of U.S. Citizenship - *only needed for applicants who need to extend the validity of their current passport - You may prove U.S. Citizenship with any one of the following:
- Certified US Birth certificate issued by the city, county or state
- Previous valid U.S. Passport (mutilated, altered, or damaged passports are not acceptable)
- Consular Report of Birth Abroad or Certification of Birth
- Certificate of Citizenship or Naturalization
- Present Proof of Identity - You may prove your identity with any one of the following, if you are recognizable:
- Previous U.S. passport (mutilated, altered, or damaged passports are not acceptable)
- Naturalization Certificate
- Current, valid
- Driver's license
- Government ID: city, state or federal
- Military ID: military and dependents
- Authorization Letter – Please download the Authorization Letter and complete two copies of the letter (according to the information contained in your passport), and sign both copies where indicated.
- 48-Page Passport - If you travel extensively, you may submit a signed request for a 48-page passport. There is no additional cost for this.

B - HAND IN/SHIP APPLICATION & RELATED
DOCUMENTS TO GLOBAL VIP SERVICES
Post the following items to us at the address below via FedEx overnight delivery service:-
- Order form (completed and signed) - original
- Your recent signed expired passport - original
- Completed and signed Application Form DS-82 - original
- Proof of Departure – copy x 2
- Two color passport-type photographs
- Proof of U.S. Citizenship * - original
- Letter of Authorization (completed and signed) – original x 2
- If you have had a name change - your Marriage/Divorce certificate or Court Order - original
- Your driver’s license - a clear copy x 2
- 48-page passport request (if applicable)- original x 2
- Payment:
- Government Fee – A check for $170.00 made
payable to U.S. Department of State
- Complete your credit card details on the Order
form, or enclose one company check, Money Order made
payable to GLOBAL VIP SERVICES for $100,
provided that you include a return mail FedEx airbill,
otherwise your payment to Global VIP Services should
include the FedEx Return Mail Fee as well.
