PART A - PREPARING APPLICATION AND SUPPORTING DOCUMENTATION

  1. Complete U.S. Passport Application Form DS-82 online.We can only accept online DS-82 applications – Click here to download DS-82 Form

    • Complete details of your travel itinerary/tickets in “Travel Plans” section.
    • Please complete “Emergency Contact” section. This is extremely helpful should you have any difficulties overseas and need assistance from a U.S. Embassy or Consulate.
    • Print a copy of the completed application form.Sign and date it.

  2. Current Valid Passport – Submit your signed current valid passport. This will be returned to you with your second passport.

  3. Proof of Departure – Your original travel itinerary or a copy of your travel ticket. In order to expedite your passport application, your travel must start within 2 weeks from the date of application.

  4. 2 Color photographs – Photograph Specification.

  5. Statement Requesting a Second U.S. Passport Click here to download the request statement, complete and sign it.

  6. Present Proof of Identity - You may prove your identity with any one of the following, if you are recognizable:

    • Previous U.S. passport (mutilated, altered, or damaged passports are not acceptable)
    • Naturalization Certificate
    • Current, valid

      1. Driver's license
      2. Government ID: city, state or federal
      3. Military ID: military and dependents

  7. Authorization Letter – Please download the Authorization Letter and complete two copies of the letter (according to the information contained in your passport), and sign both copies where indicated.

  8. Fees
    1. Government Fee $170 –You should pay this with a personal check, company check or Money Order - payable to "U.S. Department of State. (write your full name and Date of birth at the top of the check)
    2. Our Service Fee $100.00 – A money order or cashier's check made payable to “Global VIP Services”
    3. FedEx Return Mail Service – See Order Form for details

PART B - HAND IN/SHIP APPLICATION & RELATED DOCUMENTS TO GLOBAL VIP SERVICES

Post the following items to us at the address below via FedEx overnight delivery service:-

  • Order form (completed and signed) - original
  • Your recent signed current valid passport - original
  • Completed and signed Application Form DS-82 - original
  • Proof of Departure – a clear copy
  • Two color passport-type photographs
  • Statement requesting a second U.S. Passport –original x 2
  • Letter of Authorization (completed and signed) – original x 2
  • Your valid driver’s license - a clear copy
  • Payment:

    • Government Fee – A check for $170.00 made payable to U.S. Department of State
    • Complete your credit card details on the Order form, or enclose one company check, Money Order made payable to GLOBAL VIP SERVICES for $100, provided that you include a return mail FedEx airbill, otherwise your payment to Global VIP Services should include the FedEx Return Mail Fee as well.
