PART A - PREPARING APPLICATION AND SUPPORTING DOCUMENTATION
- Complete U.S. Passport Application Form DS-11 online. We can only accept online DS-11 applications – Click here to download DS-11 Form
- Complete details of your travel itinerary/tickets in “Travel Plans” section.
- Please complete “Emergency Contact” section. This is extremely helpful should your child have any difficulties overseas and need assistance from a U.S. Embassy or Consulate.
- Print a copy of the completed application form. Do Not sign this form until you are in the presence of a “Application Acceptance Agent” or County court Clerk, and instructed to do so – see Part B.
- Proof of U.S. Citizenship (applicant) - Prove of U.S. Citizenship with any one of the following:
- Certified US Birth certificate issued by the city, county or state
- Previous fully valid U.S. Passport (mutilated, altered, or damaged passports are not acceptable)
- Consular Report of Birth Abroad or Certification of Birth
- Certificate of Citizenship or Naturalization
- Present Evidence of Child's Relationship to Parents/Guardians - - You will need to submit one of the following: (Previous U.S. passports are not acceptable as proof of relationship)
- Provide Parental Identification – Each parent or guardian must submit one of the following:
- Valid Drivers License
- Valid Official U.S. Military ID
- Valid U.S. Government ID
- Valid U.S. or Foreign Passport with recognizable photo (mutilated, altered, or damaged passports are not acceptable)
- Naturalization/Citizenship Certificate from BCIS with recognizable photo
- Alien Resident Card from BCIS
- Proof of Departure – Your original travel itinerary or a copy of your travel ticket. In order to expedite your passport application, your travel must start within 2 weeks from the date of application.
- 2 Color photographs – Photograph Specification.
- Present Proof of Identity -If a child has any photo ID it should also be presented (school ID) - Original
- Authorization Letter – Please download the Authorization Letter and complete two copies of the letter (according to the information contained in your passport), and sign both copies where indicated. Parent(s)/guardian(s) must sign both copies where indicated and indicate relationship to the child next to your signature.

PART B - APPLICATION ACCEPTANCE FACILITY – PROCEDURE
- Each minor child shall appear in person at the Application Acceptance Facility.
- All applications for children under 14 require both parents or, legal guardians consent.
Present
the following items to the authorized agent at a Application
Acceptance Facility near you for approval and
let the agent know that you are using an expediting
service to "hand carry" your application. You must
pay $25 application execution
fee to the Application Acceptance Agent.
- Completed Application Form DS-11 - original
- Proof of U.S. Citizenship - original
- Proof of relationship to child - original
- Proof of child’s Departure - copy
- Two color passport-type photographs
- Letter of Authorization (completed and signed)

- Government Fee – A check for $140.00 made
payable to U.S. Department of State
- Parental Proof of Identity - copy (a clear copy of parent’s drivers license)
- Statement of consent from the second parent (if necessary) notarized & signed - original
Parent(s)/legal guardian(s) will be asked to show proof of identity (original) and the agent will ask you to sign the child’s passport application form. Children 14 years of age or older also sign the application. Please make sure that the agent notes on the application the type of ID you have presented. The agent will place the above items in an envelope and seals the envelope. The sealed envelope will be marked “To Be Opened by U.S. Passport Office Only”. Do not open this sealed envelope or your application will become invalid.
The agent must return the sealed envelope to you.

PART
C – HAND IN/SHIP PASSPORT APPLICATION & DOCUMENTS
TO GLOBAL VIP SERVICES
Post the following items to us at the address below via FedEx overnight delivery service:
- Order form (completed and signed) - original
- The Government sealed envelope.
- Letter of Authorization (completed and signed by parents/guardians) - original
- Completed Application Form DS-11 - copy
- Proof of child’s U.S. Citizenship - copy
- Proof of relationship to child – copy
- Both Parents/guardians driver’s license – a clear copy
- Proof of child’s Departure - copy
- Payment – Complete your credit
card details on the Order form, or enclose a money
order or, cashier's check made payable to GLOBAL
VIP SERVICES for $100,
provided that you include a return mail FedEx airbill,
otherwise your payment to Global VIP Services should
include the FedEx Return Mail Fee as well.
